Elderly people are characterized by degenerative changes in the structure and function of organs and tissues. The most important changes in the body composition are related to water, protein and fat balance. These changes modify the volume of distribution. A downward tendency of the volume of distribution increases the initial peak plasma concentration and the site of action. Physiological changes over the years are generally produced in a predictable manner. Individual characteristics and variabilities play an important role in the speed or rate of aging. This speed or rate for different specific physiological functions regularly and linearly declines although, there are other functions that relatively remain unchanged and only decline at the end of life or when a disease develops. Knowledge of the physiology of aging reveals the importance of rational drug use in the elderly and permits its practice in this group of the population. This work reviews the physiologic changes that affect the pharmacokinetic profiles on the basis of which a better therapeutic scheme could be developed to handle the complicated health problems of the elderly population with an eye to adapting the drug to the patient and not viceversa. © 2016 Nova Science Publishers, Inc.
